Required LSTA Signature forms

Complete documents 1-4 and submit to nslaprlsta@admin.nv.gov.

Do NOT use 2021 forms
One set of LSTA signature documents is required per Library Jurisdiction per LSTA Grant Year.

1. LSTA Assurances and Certifications

The Children's Internet Protection Act (CIPA) was enacted by Congress in 2000 to address concerns about children's access to obscene or harmful content over the Internet.

Note: NSLAPR also has an internal risk assessment tool that is completed by NSLAPR staff, using information you provide in #4, Risk Assessment Worksheet. Final results will be used to develop a project monitoring action plan (if appropriate).
