Required LSTA Signature Forms

Complete documents 1-4 and either upload to Community Grants or submit to nslaprlsta@admin.nv.gov.

One set of LSTA signature documents is required per Library Jurisdiction per LSTA Grant Year.

Note: NSLAPR also has an internal risk assessment tool that is completed by NSLAPR staff, using information you provide in #4, Risk Assessment Worksheet. Final results will be used to develop a project monitoring action plan (if appropriate).
